NAE4-HA
EXCELLENCE IN 4-H AFTERSCHOOL PROGRAMMING
Sponsored by the JCPenney Afterschool Fund

This award recognizes NAE4-HA members for excellence in development and implementation of after-school programming or providing enrichment to ongoing programs through curriculum and/or training for staff. The award is sponsored by the JCPenney Afterschool Fund.

Award

Four national awards of $500 each will be given, plus a plaque, to be presented by a representative from the JCPenney Afterschool Fund at the awards banquet at the annual session.  Winners must be present at the annual meeting.

Eligibility

1.  The award may be entered by either a collaborative team or an individual.

2.  The applicant should have successfully conducted the program or activity within two years prior to
     June 1, 2004.

3.  After-school programming includes times when youth are not in school and parents need care for
     their children;  reaches youth K-12th grade, is sustained and ongoing;  and provides holistic youth
     development to youth in after school programs.

4.  National winners must present a workshop or poster session at a national meeting.

5.  Application deadline June 10, 2004.

Application should include:

1.  Complete Member Recognition Application Form 
     (located under Awards and Recognition at nae4-Ha.org.)

2.  Program outline—limit to three double-spaced typed pages.

A.  Introduction—program objectives, target audience described.

B.  Program description - content, delivery methods described, materials developed, evidence of training offered to staff, if applicable.

C.  Programming impact - evaluation methods, results/impacts, marketing, and publicity.

D.  Support materials - limit to 5 pieces.

3. Success Story - limit to two double-spaced typed pages.

- Provide a story that highlights the impact of your 4-H Afterschool work.  The success story should focus on the impact the program had on youth and families.  Try to identify stories that have a human touch.  Testimonials and case studies can be used to illustrate the impact of 4-H Afterschool.  Be sure to have written permission for the use of statements, names and photos of youth and families.
